Mortgage Checklist

There are a number of items you will need to bring with you when you meet with your Bank of the Ozarks mortgage loan professional to proceed with your loan application process. Please use the list below as a guideline for items you may be asked to provide:

Photo ID
Social Security Number
Residence Address for past two years
Your W2s for the past 2 years
Copy of your most recent pay-stubs covering 30 days with year to date total
Copy of your last 2 months bank statements for all accounts, including investment accounts along with the address and account numbers
Copy of your last 2 years corporate tax returns, signed and dated, including all schedules (if applicable)
Balance and monthly payment for all of your current loans and the lender's name
If self employed - last two years' personal tax returns, signed and dated, including all schedules and K1’s
Loan information and address of real estate owned
Copy of Offer and Acceptance contract signed by all parties
